Meritan, Inc., a Tennessee-based health, human, and social services nonprofit, is seeking a compassionate candidate to join its Home and Community-Based Services team in Memphis, TN as a Case Manager. This full-time position provides case management services to home care clients and their families who live in the Shelby County area. Case manager duties include but are not limited to, managing a caseload, coordinating client intake, report generation, client referrals to other community resources for services, monitoring the client’s home for their safety and well-being, and providing occasional personal care services. The case manager will attend weekly case conference calls and rotate on-call once a month. Meritan continues to work through the healthcare crisis with appropriate infection control measures and personal protective equipment.

Qualifications

Minimum of a Bachelor’s degree in Social Work or a related field from an accredited college or university.

Experience in budgeting and program development preferred.
Salary : $35,000 - $37,000
