What are the responsibilities and job description for the Care Coordinator position at Meritus Medical Center Inc?
Care Coordinator (Aftercare Specialist) will provide case management and safe discharge planning for patients.
Education – High School Degree required, Bachelor’s degree in human services related field preferred
Experience – Two years of experience in mental health
Licensure/Certification – N/A
Knowledge/Skills/Abilities – Basic to intermediate computer skills required. Must be organized and assertive. Ability to work with all ages. Vast knowledge and understanding of Electronic Health Records. Effective communication skills. Ability to work in a fast paced environment. Ability to work independently and to carry out assignments to completion within parameters of instruction given.
