What You’ll Do :
Coordinate psycho-social care for patients throughout their acute episode of care.
Develop personalized discharge plans to ensure smooth transitions post-hospitalization.
Assess and address patients' needs related to care coordination, resources, and services.
Collaborate with an interdisciplinary healthcare team to improve patient outcomes and satisfaction.
Facilitate access to community resources to support patients after discharge.
Ensure compliance with evidence-based practices, regulatory requirements, and hospital policies.
What You Need to Succeed :
Education : Bachelor's degree in Social Work (Master’s preferred).
Experience : At least two years of experience in acute care, skilled nursing facilities (SNF), physician offices, or community health settings.
Licensure / Certification :
Minimum LBSW license in Maryland.
Must obtain care / case management certification within 5 years.
Skills & Abilities :
Strong communication and multi-tasking skills.
Knowledge of evidence-based practices, disease processes, and regulatory requirements.
EMR experience preferred; ability to navigate electronic medical records.
Familiarity with the revenue cycle, community resources, and provider networks.
Ability to build relationships with community partners and healthcare professionals.
