What are the responsibilities and job description for the Direct Support Provider position at Merry Glen Home?
Specific Duties of All Shifts:
· Check daily schedule for latest staffing information and group assignment(s). Attend report with outgoing shift for latest conditions of the Loved Ones and activity schedules.
· Perform physical rounds of the facility to monitor health and safety of the Loved Ones. Work closely with the team regarding areas of concern (illness, skin integrity, distress/anxiety, equipment in need of repair, etc.) or anywhere circumstances can be improved in the home.
· Assist the Loved Ones with their daily activities and hygiene according to their needs including toileting, bathing, dressing, grooming, oral care, transfers and repositioning for wheelchair users, taking meals, etc. Develop strong knowledge in the specific support approaches for everyone; examples include but are not limited to preferred routine for showering, safe transferring as recommended by the physical therapist, cutting food to proper size for eating, etc.).
· Prepare Loved Ones for any planned outings, doctor appointments, etc.
· Familiarity with each Loved One’s care plan, individual instruction and support plan and if applicable, behavior support plan. Assist them in achieving their personal goals for increasing independence and behavioral challenges such as problem solving, using communication to express their needs and feelings, anger management, etc.
· Support the Loved Ones to engage in activities they enjoy, on the grounds and around their neighborhood and community at large. Examples include but are not limited to socializing with housemates, listening to music, making crafts, gardening, playing a game, taking a walk to the park, visiting a family member or neighbor, going for coffee, etc.
· Cooperatively support family/guardian involvement and relationships with their Loved Ones, as well as professional communication and information sharing in compliance with HIPAA.
· Assist each Loved One to perform (or if they cannot, perform on their behalf) personal housekeeping chores including changing bedding, bedroom cleaning, floors, laundry, etc.
· Perform general cleaning tasks in common areas of the facility including bathrooms, mopping, wiping tables, laundry, keeping hallways clear, deep cleaning projects, etc.
· Accurately and thoroughly chart Loved Ones’ activities, care, observations and other events as required by their plans and contractual obligations via electronic tracking tool. On rare occasions, paper logging may be necessary.
· Documentation and mandatory reporting incidents may be necessary (e.g., GER, calling in to RCS, etc.). Notify off-duty leadership, regulatory agencies and law enforcement, as necessary, of issues/problems as required by contract and law.
· Assist in other duties as needed and/or assigned.
Job Types: Full-time, Part-time
Pay: $20.75 - $23.25 per hour
Expected hours: 16 – 40 per week
