We are seeking an experienced Nurse-RN to join our team as a Patient Care Coordinator. In this role, you will be responsible for performing comprehensive assessments of patients' needs, developing individualized plans of care, and collaborating with interdisciplinary teams to ensure seamless delivery of services.
You will administer medications and treatments as prescribed by physicians, while also ensuring that all necessary documentation is completed accurately and timely.
As a Patient Care Coordinator, you will delegate tasks and supervise staff to ensure effective implementation of care plans, while also maintaining open communication with colleagues and leadership to address any concerns or issues.

Qualifications
- Education and Licensure
- CPR Certification
To be successful in this role, you must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
In addition, CPR certification is required for this position.
