Job Summary:
- The Nurse Navigator is a health care professional who coordinates critical patient needs and treatment between the patients provider and other health care facilities and services to ensure the patient receives quick, appropriate services. The Nurse Navigator helps patients understand their diagnosis, treatment options and the resources available to them. The role plays a vital role in healthcare by serving as the link between the patient and the healthcare system. Facilitates a "shared goal model" within and across settings to achieve coordinated high-quality care that is patient and family centered. Provides a coordinated, strategic approach to detect early and manage effectively the chronically ill patient population. All other duties as assigned.
Qualifications:
- Associate Degree in Nursing Required; VSN or Master's program in Nursing preferred.
- Prior experience in caring for chronic disease patients required. 3 to 5 years of nursing experience in clinical or community health settings preferred. Previous Care Coordination, Case Management or Home Health experience preferred.
- Current license as a registered nurse in the State of Nevada required. BLS required.
Education
Required- Associates or better in Nursing
Licenses & Certifications
Required- L-NV-RN License
