Job Summary:
The Social Worker is responsible for in-house case work to include the resident, the family, and community resources, utilizing the psycho-social dimensions.

Job Duties/Responsibilities:
-
Provide in-house casework to residents and their families as social and emotional problems or needs arise
-
Complete referrals for various services such as hospice, dental care, behavioral health needs etc
-
Help residents and families to understand, accept, and aid in following recommendations planned to restore residents to optimum social, emotional, and physical adjustment within their capacity
-
Assess residents' social needs, participate as a team member in devising resident's plan of care, and lead care planning meetings on behalf of residents with staff from this facility and/or other community agencies
-
Prepare Social Service Initial Evaluations, keep Social Service Progress Notes, and Discharge Plan on each in-house resident, including the nature of social needs and how they are being met
-
Required to complete sections C, D, E, Q and V in MDS 3.0. Required to complete MDS interview for MDS 3.0 as scheduled by RNAC. Complete care plans for each resident.

Participate in planning facilities programs, policies and procedures; observe current policies and informs administration of gaps in programs
-
Involved in admission procedures and orientation of residents and their families
-
Provide information and referral service to the resident as a part of discharge casework
-
Participate in determining appropriate level of care for residents
-
Advocate for patients' rights
-
Keep up to date with legislation, trends, ideas, resources, and facilities related to field
-
Lead a Support Group, as needed, for the community and families of residents
-
Impart knowledge regarding long-term care, the aging process, and the complications of aging to various community groups and staff, including Nursing Assistants
-
Work in collaboration with Clinical Managers, Activities, and the Pastoral Ministries staff
-
Follow emergency procedures and respond rapidly and appropriately to emergencies ensuring the safety of the residents and staff
-
Follow proper procedures and promotes safe work practices in order to maintain a safe working environment

Work cooperatively and productively with others, including residents, families, and other employees
-
Willingness to work under direction and supervision
-
Confidentiality regarding resident information must be maintained in strict confidence, as well as other information pertaining to the business of Messiah Lifeways

Listen to and assist residents/families in the completion of grievance/concern forms
-
In conjunction with the Social Services Manager, facilitate resolutions of grievances/concerns related to lost/missing items
-
Review form for completion of information
-
Notify appropriate departments to ensure family/residents are contacted
-
Review grievance/concern form with Nursing Home Administrator
-
Log grievance/concern form
-
Sign/date form and forward to next person after appropriate follow-up

Additional duties and tasks as assigned by the Social Services Manager
Qualifications
Required Skills/Abilities:
-
Sincere desire to work with individuals who may be frail and aged
Education/Experience:
-
Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and
-
One year of supervised social work experience in a health care setting working directly with individuals.
Salary : $24 - $27
