QUALIFICATIONS:
- Valid RN license in GA (Required)
- Valid Driver’s License (Required)
- Minimum of 1 year of experience in case management or related field
- Familiarity with hospice care principles and practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical records systems
- Understanding of HIPAA regulations and patient confidentiality guidelines
DUTIES AND RESPONSIBILITIES:
- Provide case management services to patients in SNF, ALF, PCH and Pt's home
- Assess patient needs and provide care as prescribed in the Plan of Care
- Coordinate and collaborate with interdisciplinary healthcare team members
- Monitor patient progress and adjust care plans as necessary
- Conduct regular patient assessments and evaluations
- Educate patients and their families on disease management and options
- Facilitate communication between patients and healthcare providers
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
- Complete evaluations, assessments, and treatments in accordance with policy and the care plan
- Participate in the on-call rotation
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Attend and participate in bi-weekly Interdisciplinary Team Meeting (IDT)
- Maintain continuity of patient care in a hospice setting
