What are the responsibilities and job description for the Health & Family Service Specialist (Region A) position at MET Opportunities?
- Locate and maintain professional relationships with service providers and community partners within the project area and surrounding communities.
- Maintain contact with and provide support to all assigned centers to facilitate services to enrolled families.
- Identify human services available to parents and children; help parents access identified services and provide follow-up to assure an effective referral system.
- Attending parent and center committee meeting to identify and address community needs.
- Meet regularly with coordinator, operations manager and management staff to exchange information and assure implementation of Performance Standards.
- Conduct extensive outreach efforts and serve as an advocate for enrolled families and children.
- Ensure the health and well-being of pregnant women to prenatal and early childhood development.
- Oversee past and current health history of each enrollee and ensure staff follow-up on needed services.
- Assist staff in identifying medically and physically challenged children in the program, initiate individualized plans, and maintain appropriate records. Work closely with Special Services Coordinator and community partners to ensure inter-agency agreements are developed and delivered.
- Ensure all child-related health screenings occur. Provide information and support on these issues to children, families, and staff. Analyze data to assist in program planning and training.
- Work with classroom staff on following interagency referral processes for children at risk in SEL development.
- Track ASQ:SE and ASQ data in Child Plus.
- Participate in ESAXC and HSAC to provide programming input.
- Assist in meeting agency non-federal share matching requirements.
- Maintain confidentiality of program records and participant data.
- Perform all other duties as outlined by supervisor.
