Job Summary
The Registered Nurse in Home and Community Based Services provides nursing and personal care to patients in their homes, as prescribed by their physician or authorized by state/program regulations. This role coordinates with other healthcare professionals to ensure continuity of patient care and achieve optimal health outcomes.
Key Responsibilities:
- Provides high-quality clinical services within the scope of practice and infection control standards.
- Conducts clinical nursing assessments and ensures patient eligibility and medical necessity for services.
- Develops and revises individualized plans of care and service plans in collaboration with other community providers.
- Ensures plan of care frequency and duration meets patient needs and initiates revisions as necessary with physician approval.
Requirements:
- Current RN licensure in the state of practice.
- Valid driver's license, vehicle insurance, and access to dependable transportation.
- Current CPR certification.
