Job Summary
The Registered Nurse in Home and Community Based Services delivers high-quality patient care in their homes, as prescribed by physicians or authorized by state/program regulations.
This role requires coordination with other healthcare professionals to ensure continuity of patient care and achieve optimal outcomes.
Specific Job Duties/Responsibilities
- Provide clinical services within scope of practice, adhering to infection control standards and the plan of care/service plan.
- Conduct clinical nursing assessments according to federal/state program requirements and payer specifications.
- Evaluate patient eligibility and medical necessity for services as defined by payer source and agency policy.
- Develop and revise individualized plans of care/service plans with other community providers, meeting federal/state program requirements.
- Ensure plan of care frequency and duration meets patient needs, initiating revisions as needed with physician approval.
License Requirements
- Current RN licensure in state of practice.
- Valid driver's license, vehicle insurance, and access to dependable transportation or public transportation.
- Current CPR certification required.
State Specific Requirements
- LA: At least one year of clinical experience as a Registered Nurse. Recent LPN experience may be considered for Registered Nurses without one year of clinical experience.
