Job Summary
We are seeking a skilled Physical Therapist to join our team as a PRN Home Health Physical Therapist. In this role, you will provide high-quality, patient-centered care by managing the complete process of therapeutic care for assigned patients in coordination with physicians and the patient care team.
Key Responsibilities:
- Provide purposeful and skilled therapeutic care at patients' homes using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
- Instruct patients/family/caregivers in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.

- Bachelor's degree in physical therapy or related field.
- Licensed physical therapist with current certification.
