Job Overview
We are seeking a compassionate and dedicated Care Coordination Assessment Specialist to join our team. Under limited supervision, this position allows the opportunity to travel to contracted facilities to provide various health assessments to health plan members as requested by medical insurance providers. Provide medical assessment findings to medical plan providers using appropriate EMR systems. Requires strong interpersonal skills, a deep understanding of interdisciplinary care plans. The Care Coordination Assessment Specialist will facilitate access to services, provide support, and ensure that clients receive the comprehensive care they need.
Duties
- Conduct Health Risk Assessments to determine client needs.
- Provide information for assessment procedure to patient in order to gain understanding and obtain consent.
- Collaborate with multidisciplinary teams to coordinate services for clients with developmental disabilities.
- Remaining attentive to patients' comfort and safety needs.
- Maintain accurate medical records and documentation.
- Observing patients, documenting notable conditions and reactions.
- Participating in required training sessions and Care Plan Meetings with both internal and external parties.
- Advocate for clients' needs within the healthcare system and community resources.
- Participates in mandatory training and Care Plan Meetings with internal and external parties.
Requirements
- Certification in Health Services/Healthcare field or Diploma or Degree in a Human Services and/or Science or Health discipline.; relevant certifications are a plus.
- Five or more years of care coordination and/or case management related experience is preferred.
- Strong organizational skills with the ability to manage multiple cases effectively.
- Excellent communication skills, both verbal and written, to interact professionally with clients and stakeholders.
- Ability to maintain confidentiality and adhere to ethical standards in all interactions.
