Position Overview

As a Behavioral Health Specialist, you will provide essential behavioral health services under the Program of All-Inclusive Care for the Elderly (PACE) model. This position emphasizes collaborative care planning, therapeutic interventions, and advocacy within a compassionate and interdisciplinary environment.

Key Responsibilities
Behavioral Health Assessment & Care Planning:

• Conduct comprehensive behavioral health assessments to identify participant needs and preferences.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.

Therapeutic Intervention Delivery:

• Provide interventions including symptom management, crisis intervention, counseling, and care management.
• Document all therapeutic activities in accordance with program standards and compliance guidelines.

Therapy Facilitation:

• Lead individual and group therapies using diverse formats such as:
• Art therapy
• Music therapy
• Psychosocial drama
• Counseling to foster participant engagement and social connection.

Training & Education:

• Provide staff training on recognizing behavioral health symptoms and effective intervention techniques.

Required Qualifications
Education:

• Master’s Degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD).

Licensure:

• Licensed Clinical Social Worker (LCSW) by the California Board of Behavioral Sciences or
• Licensed Psychologist by the California Board of Psychology.

Experience:

• Minimum of five years in behavioral health.
• At least two years of experience providing therapy for frail or elderly populations.

Skills & Certifications

• Certification: Active LCSW or Psychologist licensure in California.
• Therapeutic Expertise: Proficiency in behavioral health assessments, therapy facilitation, and care management.
• Interpersonal Skills: Strong communication and empathy to deliver patient-centered care.
