Join Our Award Winning Client as a Registered Nurse for the PACE Program in Los Angeles, CA. This role is ideal for a mission-driven professional passionate about delivering high-quality, integrated care to frail older adults. As a Complex Care RN, you will oversee and provide direct nursing care to PACE participants in diverse settings, including clinics, adult day centers, homes, virtual environments, and skilled nursing facilities.
As a key member of the Interdisciplinary Team (IDT), you will actively contribute to assessments and care plans, ensuring participant-centered care. This opportunity allows you to practice at the top of your license while advancing a care model that prioritizes quality, outcomes, teamwork, and empathy.
Key Responsibilities
- Actively participate in Interdisciplinary Team (IDT) meetings, contributing assessments, care plan recommendations, and care coordination.
- Conduct face-to-face nursing assessments that address physical, psychosocial, and behavioral health needs across various care settings.
- Collaborate with medical providers to deliver personalized care based on individual participant care plans.
- Administer and document nursing interventions, medication management, and therapeutic treatments as prescribed.
- Triage outpatient care, initiating therapies within scope of practice and escalating care in collaboration with medical providers.
- Provide case management during care transitions and communicate proactively with hospital and long-term care case managers.
- Educate participants, caregivers, and team members on personalized care plans.
- Participate in a rotating on-call schedule to triage after-hour nurse calls.
