Key Responsibilities
- Perform Start of Care (SOC) visits (3/day)
- Develop and manage patient care plans
- Collaborate with a multidisciplinary care team
- Ensure timely and accurate charting
- Utilize mobile devices and EMR software
- Manage daily schedules independently
- Deliver high-quality, compassionate care
Schedule and Shift Details
- Full-Time:
- Monday–Thursday or Tuesday–Friday
- 8:00 AM – 6:00 PM
- Location: Marion, IL and surrounding areas
