Qualifications

- Electronic Medical Record (EMR)

- TruChart EHR
- Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)
- ANY of the following valid licenses/certifications:
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Basic Life Support (AHA BLS)
- Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all three)?
Preferred:

Overview
Our Award-Winning Client is seeking a compassionate Home Health LPN to join their extraordinary care team in Stroudsburg, PA. The ideal candidate will have an Associate degree, an active LPN license in Pennsylvania, and two years of LPN experience. This role involves collaborating on patient care plans, supporting patients in the local area, and utilizing technology tools effectively.
Key Responsibilities
- Collaborate to manage and deliver patient care plans
- Support patients in Stroudsburg, PA, and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule efficiently
- Demonstrate experience with tablets, mobile phones, and EMR software
Schedule and Shift Details
- Schedule: Monday-Friday, 8 AM - 5 PM with one weekend per month (Saturday & Sunday, 8 AM - 5 PM)
- Weekend Adjustment: Two weekdays off will be provided to compensate for weekend hours
Home Health LPN – $10,000 Bonus
- Location: Stroudsburg, PA
- Territory/Driving Expectation: Monroe County
- Preferred Experience: Home Health/Hospice, Acute Care, LTC, SNF
- Not Preferred: N/A
- Schedule: Monday-Friday, 8 AM - 5 PM with one weekend per month (Saturday & Sunday, 8 AM - 5 PM)
