Overview
Our Award Winning Client is seeking a dedicated Home Health LPN to join their extraordinary care team in Pleasant Hills, PA. Key Responsibilities
- Manage and deliver patient care plans
- Collaborate with the care team to provide high-quality patient care
- Support patients in Pleasant Hills, PA, and surrounding areas
- Meet critical charting deadlines
- Self-manage time and schedule effectively
- Utilize tablets, mobile phones, and EMR software
Schedule and Shift Details
- Schedule: Monday–Friday, 8-hour days
- Weekend: About 1 weekend per month
Location & Travel
- City/State: Pleasant Hills, PA
- Territory/Driving Expectation: 20–25 miles surrounding the posted location
- Travel: Occasional local travel
Preferred Experience:
- Prior home health (HH) or hospice experience
- At least 1 year of medical-surgical (med/surg) experience
- Minimum 1 year of nursing experience
Excluded Experience:
- Private duty, dialysis, or other limited-scope areas
Compensation
- Pay Rate: Around $30/hr (salaried role)
