Position Overview

Our client, a leader in home health services, is seeking a compassionate and experienced Home Health Medical Social Worker (MSW) to join their care team. This role offers the opportunity to deliver patient-centered care, support individuals in their homes, and address unique challenges to improve overall patient outcomes.

Key Responsibilities

• Collaborate with interdisciplinary teams to develop and manage individualized care plans.
• Provide emotional support to patients and families, helping them navigate medical, social, and emotional challenges.
• Conduct psychosocial assessments to identify patient needs and recommend appropriate interventions.
• Educate patients and families on community resources and assist them in accessing support services.
• Deliver care to patients in Jackson, MI, and surrounding areas.
• Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system.
• Travel to patient locations (approximately 20% of the workday) to ensure continuity of care.

Qualifications
Required:

• Master’s Degree in Social Work (MSW) from a CSWE-accredited program.
• 1-2 years of post-graduate experience in social work, preferably in healthcare or home health.
• Active MSW license in the state of Michigan, in good standing.
• Valid driver’s license, reliable transportation, and auto insurance.
• Proficiency in EMR systems (e.g., TruChart EHR) and experience with mobile devices or tablets.

Schedule and Travel Requirements
