Home Health RN – Weekend
About the Role
Our award-winning client, a leader in home health care, is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality, patient-centered care during weekends. This role involves managing personalized care plans, ensuring accurate documentation, and coordinating with healthcare teams to enhance patient outcomes. The ideal candidate is self-motivated, highly organized, and dedicated to delivering exceptional in-home nursing care.
Key Responsibilities
Patient Care Management
- Develop and oversee individualized patient care plans in collaboration with healthcare teams.
- Ensure effective coordination of care to meet each patient's unique needs.
Direct Patient Care
- Provide in-home nursing care to patients in Carnegie, Crafton, PA, and surrounding areas.
- Deliver compassionate, hands-on medical support, including assessments and treatments.
Clinical Documentation
- Maintain accurate, timely records in compliance with charting and documentation standards.
- Utilize Electronic Medical Records (EMR) systems efficiently.
Time & Schedule Management
- Independently manage schedules to meet patient needs and on-call responsibilities.
- Ensure timely response to patient needs during assigned shifts.
Travel & On-Site Care
- Approximately 20% of the workday will involve traveling between patient locations.
Schedule & Shift Details
On-call hours:
- Friday, Saturday, and Sunday: 5:00 PM – 10:00 PM.
- Saturday & Sunday: Additional 8 units of care required each day.
- Holidays: On-call for two holidays per year.
Qualifications
Required:
- Associate degree or higher in Nursing (RN program).
- Valid Registered Nurse (RN) license in Pennsylvania (PA).
- At least 1 year of experience in any of the following areas:
- Clinical Documentation
- Clinical Informatics
- Clinical Quality
- At least 1 year of experience working with Electronic Medical Record (EMR) systems (e.g., Practice Partner EMR).
Certifications:
- American Red Cross Advanced Life Support (ARC ALS).
- American Heart Association Advanced Cardiac Life Support (AHA ACLS).
Preferred:
- RN License Number must be provided for verification upon application.
