Schedule
- On-Call: Friday, Saturday, and Sunday (5 PM – 1 AM)
- Scheduled Visits: Saturday and Sunday (8 units per day)
About the Role
We are seeking a dedicated and compassionate Home Health Registered Nurse to provide exceptional care during weekends. This role combines on-call responsibilities with scheduled patient visits, offering a structured and flexible environment where you can make a meaningful difference in patients' lives.
Key Responsibilities
- Care Management: Develop, implement, and manage individualized care plans in collaboration with the care team.
- Patient Support: Deliver high-quality, compassionate, home-based care to patients in California, PA, and nearby areas.
- Travel: Spend approximately 20% of your time traveling to patient locations (mileage reimbursement provided).
- Clinical Documentation: Ensure accurate and timely documentation of patient records, meeting all deadlines.
- Time Management: Balance on-call duties with scheduled visits to maintain efficiency and quality of care.
Required Qualifications
Education:
- Associate’s Degree or higher in Nursing.
Experience:
- At least 1 year of experience in:
- Clinical Documentation
- Clinical Informatics
- Clinical Quality
- Proficiency with EMR systems (e.g., Practice Partner EMR or similar platforms).
Licensure:
- Active RN license in Pennsylvania (PA).
Certifications:
- Advanced Life Support (ALS) certification (American Red Cross).
- Advanced Cardiac Life Support (ACLS) certification (American Heart Association).
Transportation:
- Reliable vehicle, valid driver’s license, and current auto insurance for local travel.
Preferred Qualifications
