Hospice After Hours Licensed Practical Nurse (LPN)
Position Overview
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide critical after-hours hospice care in Springfield, IL, and surrounding areas. Key Responsibilities
- Collaborate with healthcare colleagues to manage and implement individualized patient care plans.
- Deliver timely, compassionate care to hospice patients and their families during after-hours shifts.
- Maintain accurate and comprehensive documentation, ensuring charting deadlines are met.
- Effectively manage time to prioritize and address patient needs during scheduled hours.
- Travel to patient locations as required to provide care and support.
QualificationsRequired:
- Associate degree or higher in Nursing or a related field.
- Minimum of 2 years of hospice experience.
- At least 1 year of experience working with Electronic Medical Records (EMR) systems (e.g., TruChart EHR or similar).
- Valid LPN license in the state of Illinois.
- Active driver’s license, reliable transportation, and current auto insurance.
- Willingness to travel locally within the Springfield area.
Preferred:
- Active RN license (preferred but not required).
Schedule & Travel Details
- Shift: Monday – Thursday, 4 PM to Midnight (32 hours/week).
- Travel: Local travel within Springfield and nearby areas as needed.
Compensation
- Competitive hourly rate: Approximately $30/hour (negotiable based on experience).
