Licensed Practical Nurse – Hospice (Full-Time)
Location: Shiloh/Fairview Heights, IL

We’re seeking a dedicated Hospice LPN to join our client’s exceptional care team in the Shiloh/Fairview Heights area. This full-time role offers the chance to deliver compassionate, patient-centered care in a supportive and collaborative environment. As a Hospice LPN, you’ll work closely with an interdisciplinary team to implement care plans and ensure comfort and dignity for patients in their homes or care settings.
What You'll Do:
- Partner with clinical staff to manage and carry out individualized care plans
- Provide direct patient care and emotional support for patients and families
- Travel to homes and facilities within the local territory
- Accurately document care and meet required charting deadlines
- Organize and manage your schedule independently while staying aligned with team goals
What We’re Looking For:
Required:
- Associate degree or higher in nursing
- Valid Licensed Practical Nurse (LPN) license in the state of Illinois
- Minimum of 2 years of hospice experience
- At least 1 year of experience with any of the following:
- Electronic Medical Records (EMR)
- TruChart EHR or other EMR systems
- Active driver’s license, reliable transportation, and auto insurance
- Willingness to travel locally within the assigned service area
Preferred:
- Current LPN license number (please include when applying)
Schedule Details:
- Work schedule and specific shift expectations will be reviewed during onboarding
- Occasional travel required within Shiloh, Fairview Heights, and nearby areas
