Key Responsibilities
- Collaborate to provide management and delivery of patient care plans.
- Support patients within South Park, PA, and surrounding areas.
- Meet critical deadlines for charting and documentation.
- Effectively self-manage time and schedules for after-hours patient care.
- Demonstrate proficiency with tablets, mobile phones, and EMR software.
Schedule and Shift Details
- Typical Schedule:
- Monday–Thursday, 5 PM–8 AM.
- Alternate Option: Monday–Friday, 5 PM–1:30 AM (subject to consideration).
Travel Requirements
- Local travel is required. Candidates must have a valid driver’s license, a reliable mode of transportation, and auto insurance.
Qualifications
Required
Education:
- Associate degree or higher in Nursing.
Experience:
- Minimum 2 years in Hospice.
- 1 year experience with: Electronic Medical Records (EMR) systems (e.g., TruChart EHR or other EMRs).
Licensing:
- Valid Registered Nurse (RN) license in Pennsylvania.
Preferred
- Provide RN license number upon request.
