Key Responsibilities
- Develop and implement individualized care plans in collaboration with the hospice team.
- Provide direct nursing care to patients in New Kensington, PA, and surrounding areas.
- Maintain accurate and timely documentation to ensure compliance with medical and regulatory standards.
- Manage schedules efficiently to meet patient needs while adhering to care timelines.
- Utilize electronic medical records (EMR) systems, mobile devices, and tablets to document and access patient data.
Schedule Details
- Shift: 8:00 AM Saturday β 8:00 AM Monday
- Includes scheduled rest periods during the shift
Travel Requirements
- Local travel is required to visit patients in their homes or care facilities.
Qualifications
Required:
- Associate degree or higher in nursing
- Minimum of 2 years of hospice nursing experience
- At least 1 year of experience with:
- Electronic Medical Records (EMR)
- TruChart EHR or other EMR platforms
- Valid Registered Nurse (RN) license in Pennsylvania
- Active driver's license, reliable transportation, and valid auto insurance
Preferred:
- RN license number provided with application
