This role focuses on providing patient-centered primary care, emphasizing health promotion and disease prevention. Working in close collaboration with physicians and interdisciplinary teams, you will play a critical role in delivering high-quality care to a diverse patient population.
Key Responsibilities
- Deliver comprehensive, evidence-based care that addresses both acute and chronic medical needs.
- Conduct thorough patient evaluations, including obtaining medical histories and performing physical exams.
- Maintain detailed and timely Electronic Health Records (EHR) to ensure seamless patient care.
- Collaborate with a multidisciplinary team to provide culturally competent care tailored to individual needs.
- Refer patients to specialists or coordinate additional care as necessary.
- Participate in ongoing professional development, training, and quality improvement initiatives.
- Engage in patient education, focusing on preventative care and disease management.
Qualifications
Required:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Valid Connecticut licensure as an APRN or PA.
- Eligibility for DEA registration and state narcotic certification.
- Proficiency in clinical guidelines and delivering care in a primary care setting.
- Strong verbal and written communication skills to interact effectively with patients and healthcare teams.
- Competence in using EHR systems and other healthcare technologies.
Preferred:
- 1-2 years of experience in ambulatory or urgent care settings.
- Bilingual, with Spanish proficiency being highly desirable.
