Key Responsibilities

• Provide skilled nursing care, including wound treatment, medication administration, and patient education in a home setting.
• Conduct comprehensive assessments and develop customized care plans tailored to individual patient needs.
• Collaborate with interdisciplinary teams to ensure holistic patient support.
• Communicate effectively with patients and families, ensuring clarity and comfort throughout the care process.
• Maintain accurate documentation of all patient interactions and care activities in compliance with company and regulatory standards.
• Encourage patient independence and enhance well-being through compassionate, professional care.

QualificationsRequired:

• Associate's degree or higher in Nursing.
• Active RN license in Pennsylvania (PA).
• At least 1 year of RN experience.
• Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification (American Heart Association).
• Valid driver’s license, reliable transportation, and auto insurance.

Preferred:

• 1 years of Home Health experience.
• RN License Number (to be provided during application).

• Strong expertise in patient assessments and skilled nursing procedures.
• Excellent communication and patient education skills.
• Strong organizational abilities for managing schedules and personalized care plans.
• Proficiency with electronic health records (EHR).

Position Details

• Schedule: Full-time, Monday to Friday with flexible hours and minimal weekend duties.
• Pay Per Visit Rate: $48–$54 per visit.
• Orientation Guarantee: $3,000 per pay period for the first six months.
• Estimated Annual Earnings: $77,000–$100,000 .
