Home Health Registered Nurse (RN)

Key Responsibilities
- Assess patients and develop personalized care plans.
- Administer medications and treatments as prescribed.
- Collaborate with physicians, therapists, and other healthcare professionals.
- Educate patients and families on disease management and self-care.
- Travel to patients' homes within Pike County (mileage reimbursement provided).
- Maintain accurate patient records and documentation.
Qualifications
Required
- Associate’s Degree or higher in Nursing.
- Active RN license in Pennsylvania.
- Minimum of 1 year of RN experience (home health experience preferred).
- BLS & ACLS certifications.
- Valid driver’s license, reliable vehicle, and auto insurance.
Preferred
- Submission of RN license number with application.
Work Schedule
- Monday–Friday, 8:00 AM to 5:00 PM.
- On-call weekends every 4–8 weeks.
