Our client is seeking a compassionate and experienced Registered Nurse (Home Health) to provide personalized care in patients’ homes in the Charleroi, PA area. This position offers a flexible schedule, travel reimbursement, a generous onboarding program, and a $15,000 bonus paid quarterly.
Key Responsibilities
- Deliver high-quality nursing care to patients in their homes, adhering to individualized care plans.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Maintain accurate and timely documentation in compliance with regulatory requirements.
- Educate patients and families on disease management, medications, and care plans.
- Participate in on-call rotations and ensure continuity of care during scheduled shifts.
- Utilize clinical skills to assess, plan, implement, and evaluate patient care.
Top Skills Required
- Experience: Minimum of 1 year as an RN (Home Health experience preferred).
- Education: Associate’s Degree or higher in Nursing.
- Licensure: Active RN License in the State of Pennsylvania (PA).
- Certifications:
