- Associate degree or higher
- 1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- Practice Partner EMR
- Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)
- Valid Regular Driver's License (DL)
- ANY of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- Please provide your RN License Number.

- Collaborate with the care team

- Meet critical deadlines for charting
- Self-manage time and schedule efficiently
- Utilize tablets, mobile phones, and EMR software
- Schedule: Monday-Thursday or Tuesday-Friday, 8 AM - 6 PM (completing 3 SOC Visits/day)
- Location: Watsontown, PA
- Territory/Driving Expectation: Snyder, Union, Lycoming, Clinton, and Upper Northumberland Counties
- Preferred Experience: Home Health/Hospice
- Not Preferred: MUST have Home Health RN experience
- Pay Rate/Salary: $80,000 with a $10,000 FYS bonus
- Schedule: Monday-Thursday or Tuesday-Friday, 8 AM - 6 PM (completing 3 SOC Visits/day)
