What are the responsibilities and job description for the Re-Entry Case Manager position at Metro Community Ministries Inc.?
Case Manager will assess newly assigned participants to identify strengths and needs. Use assessment information to develop an individualized service plan. The case manager will work with individuals to create a plan of action to focus on credentialed training, job placement, and job retention strategies. The Case Manager will conduct ongoing assessment and evaluation of participants and update individualized service plan as participant meets plan goals and objectives; Ensure regular, ongoing sessions with assigned participants, designed to plan and follow through with individualized and program goals; Utilize strength-based and trauma-informed techniques in interactions and service delivery strategies; Facilitate group-based case management services; ensure initial intensive services are provided and are adequate to meet individual needs.
In person services will provided to program participants.
Job Type: Full-time
Pay: $38,000.00 - $40,000.00 per year
