In conjunction with clinical care, Metrocare’s Altshuler Center for Education & Research is transforming the community mental health landscape of North Texas through workforce development focused on innovative partnerships and exceptional trauma-informed training.
Healthy Transitions is a system of care framework that creates the infrastructure and support needed for transitional age youth to be successful. The SOC includes agencies, hospitals, residential treatment facilities, child welfare systems, schools, educational resources and supports to help families thrive. Metrocare is the anchor agency for Healthy Transitions. The Systems of Care Lead Family Coordinator is responsible for the ensuring that the system of care initiatives and the services its supports are implemented and delivered in a manner that respects all individuals served and incorporates the recommendations from those with lived experience to improve service delivery. A requirement for the Lead Family Coordinator position is for the individual to be a current or previous caregiver of a child who has a serious emotional disturbance. The Lead Family Coordinator should have a strong connection to the community and be very knowledgeable about resources, services and supports for families.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Provides peer mentoring and supports to primary caregivers and youth.
- Provides peer mentoring and support to Family Partner and Family Advocate members of Healthy Transitions.
- Collaborates with Healthy Transitions members to ensure the treatment process is a strength-based and trauma informed approach to providing care and services.
- Models’ self- advocacy skills.
- Provides training to Healthy Transitions members, board, family partners/advocates and community.
- Leads Healthy Transitions Family Partner Workgroup.
- Helps identify and then support the family member and/or family partner that serves as member of the Healthy Transitions Governance Board.
- Help identify and support family members, partners/advocates who are members of Healthy Transitions.
- Assists in the identification of natural/non-traditional and community support systems, and links Healthy Transitions to community resources that can be incorporated into and/or partner with the system of care.
- Performs other duties as assigned.

QUALIFICATIONS
EDUCATION AND EXPERIENCE:
- Required: High School Diploma or equivalent
- Required: Individuals with lived experience as caregiver of child or youth diagnosed with a mental health condition.
- English/Spanish preferred
MATHEMATICAL SKILLS:
- Basic math skills required.
REASONING ABILITY:
- Ability to apply common sense understanding to carry out simple one or two-step instructions.
- Ability to deal with standardized situations with only occasional or no variables.
COMPUTER SKILLS:
- Use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, and PowerPoint).
- Ability to utilize Internet for resources.
CERTIFICATIONS, LICENSES, REGISTRATIONS:
- Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain a Texas Driver License within three (3) months of employment.
- Liability insurance is required if an employee will operate a personal vehicle on Center property or for Center business. Must be insurable by Center’s liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
