We are looking for a motivated nurse that will work in our Adult Foster Care program and there will be opportunities for cross training across our other programs as well!

What You’ll Do:
- Member Advocate Extraordinaire: Be the voice for those navigating the healthcare maze. Unravel complex cases, connect dots, create care plans, and much more!
- Coordination Maestro: Collaborate with doctors, therapists, social workers, and the MetroCare team to ensure seamless transitions. Think of it as orchestrating a symphony of care.
- Tech-Savvy & Chart Whisperer: Our EMR system? You’ll tame it like a pro. Consider it your digital sidekick.
- Detail-Obsessed: Missed details? Not on your watch. Your eagle eyes will spot the nuances that matter.
- New Grad Program: Our nursing leadership team has curated a robust orientation program that is catered for nurses that are new to case management. Once the program is completed, you will have access to ongoing trainings as needed, refresher workshops, mentoring sessions, etc.
Qualifications:
- New graduate of an accredited LPN program, within 5 years.
- Must be able to navigate the Microsoft Office Suite (Outlook, Teams, Excel, etc.) and Adobe PDFs.
- Strong communication skills.
- Detail-oriented, organized, and committed to member-centered care.
- Massachusetts LPN nursing license (or the determination to obtain one prior the date of hire).
- Bilingual skills (English/Spanish) are a bonus. ¿Hablas Español? ¡Perfecto!
Compensation and Benefits:
- Starting between $36-38/hr
- Hours are 8am-4:30pm, no weekends!
- 8 Paid Holidays
