This is an exempt position responsible for maximizing education for patients, choosing access modality and Home Therapies. Also responsible for empowering patients to confidently choose access type with a focus on Home Therapies as their first modality option.
Duties include, but are not limited to:
- Responsible for creating and maintaining an office Home Therapies process manual as well as implementing Raenali training education
- Establishes a workflow for capture of all potential home therapies candidates at each location
- Collaborates with providers in the evaluation of patient’s access choice and potential home therapies candidates
- Identifies a team representative at each office location and educates them in home therapies using a systematic approach
- Works with VBC to develop education schedules at each location in response to patient activation levels as indicated per PAM survey response
- Conducts regular team representative meetings with a focus on patient identification, progress and resolution of patient issues
- Develops action plans for patients in terms of visit compliance (educational, home, surgical)
- Coordinates all aspects of patient care with the appropriate staff members, from time of identification to training completion
- Develops a workflow based on TOPS visit completion for provider discussion and sign off
- Develops with VBC team implementation of Raenali Education for patients
- Collaborates with VBC team on aligned VBC patient’s status of access and or home therapies choice
- Maintains and monitor a monthly patient progress report for the Director of Value Based Care Operations
- Collaborate with FMC staff to define a patient communication process which evaluates progress and eliminates duplication of efforts
- Empowers patients to recognize their ability to treat themselves in the comfort of their own home
- Provide education and support to patients and caretakers on patient’s modality choice
- Provide education and support to patients and caretakers who have chosen home therapies
- Develops a system whereby patients have direct access to the coordinator and/or team representatives
- Acts as a resource for the patient and their support system to address their concerns and questions

Education: : Graduate of an accredited nursing program.
Experience: Minimum three years of professional nursing experience preferred, as well as some supervisory level experience.
Certification: Must hold a current and valid North Carolina RN license. Current CPR certificate.
Technical Skills: Knowledge of nursing processes; health care systems, structure, and functions; and community resources. Familiarity with state law on nursing care, nursing guidelines, and clinic policies and procedures.
Management Skills: Develop plans to meet designated objectives; Create a structure to support or accomplish plans; Direct, delegate and be decisive; Maintain complete oversight of assigned locations.
