Part-Time CFSS Consultation Worker / Case Manager / Social Worker
Are you passionate about empowering individuals to take control of their care and services? A local nonprofit is seeking a dedicated Case Manager / Social Worker to provide Community First Services and Supports (CFSS) Consultation Services to individuals who require assistance to live and work independently in their community.
About the Role:
As a CFSS Consultation Worker, you will play a crucial role in educating, supporting, and guiding individuals through the CFSS program. This program offers participants greater choice, flexibility, and control over their services, similar to Personal Care Assistance (PCA) services.
Key Responsibilities:
- Educate individuals about CFSS, the agency model and budget model
- Assist in developing, reviewing, and submitting CFSS service delivery plans
- Guide individuals in choosing appropriate CFSS provider agencies or FMS providers
- Provide ongoing support, education, and consultation throughout the year
- Assist individuals with changes to their service plans, models, or providers
- Ensure compliance with DHS policies and document any complaints as required
- Conduct semi-annual reviews for individuals without case managers or care
- coordinators
- Support culturally diverse populations with a person-centered approach
Qualifications:
Education & Experience: Must meet one of the following:
- Bachelor’s degree (or higher) in a human services field
- Associate’s degree (or higher) one year of experience working with individuals with disabilities
- Equivalent experience in direct care, self-directed services, or service coordination
Additional Requirements:
- Must be 18 years or older
- Must pass a background check per Minnesota Statutes, Chapter 245C
- Strong communication and organizational skills
- Experience with CFSS, PCA, or disability services preferred
