MES provides housing case management services to underserved individuals and families by offering wrap-around services leading to housing stability and self-sufficiency. Our programs are designed to help residents with chronic homelessness transition into stable and permanent housing
Direct Reports:
The Program Manager reports directly to the Director of Case Management Services
Position Summary:
The Program Manager is directly responsible for the initiation of the family and individuals Permanent Supportive Housing case management services program. The Program Manager is responsible for ensuring each client has the opportunity to be housed from homelessness and be supported during the duration of this transition and after being housed. In addition, they must ensure that each client participates fully in relevant and appropriate social, academic, and independent living activities. The Program Manager advises and counsels the Client Services Coordinators and other members of the Leadership Team on, but not limited to, the program’s day to day operations, activities schedule and direct case management services. The Program Manager will provide primary responsibility for the supervision of Case Managers. The Program Manager will oversee intake, assessment, assignments, treatment plan development, documentation, discharge summaries and post-release planning. In addition, the Program Manager will assist the Director of Case Management Services in the provision of administration and training of staff.

Essential Functions:
Supervisory
Manage a team of 10-15 case managers
Oversee and manage ongoing staff training to maintain compliance with contract requirements
Lead a team office day, including checking QuickBase note compliance and client contact requirements for case managers. Meet with case management staff daily at Office Days to ensure staff effectiveness in service delivery
Conduct weekly Team Meetings
Manage PTO and leave requests, with the assistance of the Director of Case Management Services, to ensure daily functioning is not disrupted.
Monitor staff deliverables, including timeliness, task management, client contacts, etc. and support disciplinary action measures as approved by the Director of Case Management Services
Provide direct monthly supervision to Team Leads and Case Managers
DHS Communication
Serve as direct contact to our partner agencies
Serve as direct contact to DC Department of Human Services for all client-related items
Attend DHS meetings including one-on-ones with DHS Monitors, monthly Provider Meetings, ICH Meetings, etc.
Lead the client exit process and submit required forms to DHS, as needed
Administrative/QA
Pre-screen and interview new case management staff
Maintain an active roster of case management staff
Conduct orientation and administrative processing of new case managers
Assist in the implementation of the comprehensive quality assurance system which is consistent with the overall goals and objectives
Monitor charts on each client to ensure compliance
Monitor Quality Assurance process for client services via other members of the Leadership Team to discuss current goals and services being provided
Monitor submission of Home Visit, Child Visit, Utility Bill payment forms
Attend Treatment Team meetings as scheduled
Advocate for clients with referral and service agencies
Complete monthly case management reports of clients and maintain current documentation of records
Ensure development and monitor Individual Service Plans and Assessments for each client
Perform other duties as assigned
Minimum Qualifications:
Bachelor’s degree in social work, psychology, sociology, and counseling or other related social service/science disciplines required
Master’s degree in social work, psychology, sociology, and counseling or other related social service/science disciplines preferred
Licensed Clinical Social Worker (LICSW) or a Licensed Professional Counselor (LPC) preferred
Must have a minimum of three (3) years of experience supervising case managers
Must have excellent motivation, communication, writing, organization and computer skills
Must be able to support the company vision
Successfully pass Police, Child Registry and Federal Background Clearance Checks
Working Conditions:
This is a full-time position, Monday through Friday, 9:00 a.m. to 5:00 p.m., Office-based position. Occasional evening and weekend work may be required as job responsibilities demand.
Physical Requirements:
While performing the duties of this job, the employee will frequently sit, stand, walk, drive, and reach—prolonged periods of driving, walking, and possible meetings.
