Typical Duties
- Engage with MPHD clinic customers and Davidson County residents to assess their health and social service’s needs.
- Use care coordination and resource navigation tools and skills to refer customers and residents to needed resources, aid in-clinic appointment setting, and follow up to make sure need is addressed.
- Use health assessment tool the Protocol for Responding to & Assessing Patients’ Assets, Risks & Experiences (PRAPARE) with customers and residents and makes sure collected information is entered in initiative database.
- Provide one-on-one and group health education and establishes positive, supportive relationships with customers and residents receiving education.
- Completes CHW training as assigned.
Minimum Qualifications
Candidates with accreditations earned in a foreign institute are encouraged to apply, however at the present time MPHD is not sponsoring individuals through any of the US Government’s employment-based immigration programs.

Preferred Experience, Knowledge, Skills, and Abilities
- At least one (1) year of full-time employment in any public health, health, or social services field. Must have trust and experience with the community served including knowledge and feel connected to the community by having shared experiences and a desire to help the community.
- Self-motivated and task oriented
- Be responsible, dependable and the capacity to perform multiple tasks.
- Bilingual Spanish, Arabic or Swahili
