This position is eligible for our sign on bonus of $1000.
The Patient Services Coordinator is an integral position that works closely with the clinic Medical Home care team to manage the comprehensive needs of our complex patient population. The diverse responsibilities can include, but are not limited to, timely care coordination through direct contact with patients and their families, processing and tracking referrals and prior authorizations, patient education, and community outreach.
Essential Functions
- Engage with families/patients through routine outreach utilizing phone contact, scheduled and unscheduled face-to-face meetings before, during, and after medical appointments.
- Process referrals and prior authorizations in a timely manner and ensure that the referral is documented in the EMR system, communicated, and complete.
- Participate in team meetings sharing patient referral information with the care team to create effective care.
- SYNAGIS® coordination and management.
- Identify and decrease the impact of barriers interfering with a patient’s health and well-being, including financial limitations, cultural backgrounds, and parental mental health issues.
- Actively maintain the referral portion of the external physician and clinic database.
- Successfully work in a fast-paced, changing environment. with effective time management, ability to multi-task and prioritize workload.
- Participate in site-specific and clinic-wide initiatives on an ongoing basis.
- Attend and actively participate in huddles and team meetings.
- Sustain excellent patient interactions using clear communication and problem-solving skills.
- Always maintain confidentiality of sensitive patient information.
- Display high standards of office conduct.
- Punctual, regular, timely, and dependable attendance.
Qualifications
Education/ Experience
- High school education or equivalent.
- 3 years of experience in a clinical setting required.
- Experience with EMR required, preferably Epic.
- Understanding of PCPCH (Patient-Centered Primary Care Home) requirements.
- Knowledge and understanding of medical terminology and clinical procedures.
- Minimum of 3 years of experience in customer service, computers, general office procedures, multitasking, and understanding of insurance terms.
Salary : $20 - $25
