Job Summary:
The Social Worker facilitates care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. The individual assesses patients for transition needs, identifies those at risk for readmission, and conducts complex psycho-social assessments and interventions.
Main Responsibilities:
- Complex psycho-social transition planning, assessment, and intervention
- Assistance with adoptions, abuse and neglect cases, including assessment, intervention, and referral
- Care coordination and implementation or oversight of transition plans
- Leading and/or facilitating multi-disciplinary patient care conferences
- Making referrals to other departments and communicating with patients and families about the plan of care
Requirements:
- Master's of Social Work degree required
- At least 2 years of acute hospital experience preferred
- Licensed or license eligible to practice as a LSW or LCSW in accordance with state regulatory requirements
