Care Coordinator Job Summary:
We are seeking a Care Coordinator who will coordinate the care needs of assigned patients and develop comprehensive plans to manage care delivery across the patient care continuum.
The successful candidate will partner with patients and their primary physicians to develop customized care plans based on individual needs and preferences.
- Responsibilities:
- Collaborate with physicians, nurses, allied health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion.
- Advocate for patients and balance care needs and financial considerations to ensure efficient and effective treatments are achieved.
Qualifications:
- Education:
- Academic degree in nursing (Required), Bachelor's or master's degree (Preferred).
- Experience:
- At least 2 years of nursing experience (Required).
- Certifications:
- Current licensure, certification, or registration to practice profession as required by law, regulation in state of practice or policy; CPR (Required).
