Description
I) Job Purpose
Certified Community Health Workers (CCHWs) are trusted frontline workers who bridge the gap between communities and health / social services. They empower individuals and communities by providing outreach, education, counseling, and advocacy. CCHWs engage marginalized populations, including those with mental illness, substance use disorders, and homelessness, using relational theory and stages of change concepts. Their efforts, aligned with MHMR Tarrant's mission, aim to reduce health disparities, and improve access to care. Known outcomes include increased access to behavioral health services, better health outcomes, enhanced communication between communities and health providers, and reduced reliance on emergency services, in line with CARF and CCBHC standards.
II) Essential Functions
A) Provide integrated outreach to high-risk individuals with behaviors suggestive of mental illness, substance use disorders, marginalized health disparities, and more.
B) Conduct daily outreach to high-risk individuals and perform weekly follow-ups as per CoC guidelines. High-risk outreach should occur 60%-90% within the community.
C) Maintain frequent communication using various methods like in-person, calls, texts, and letters, collaborating with community partners for effective outreach.
D) Mentor contacts in using service providers, assist with transportation, harm reduction, empowerment techniques, and liaison services.
E) Connect contacts to required IOS program services, self-help groups, and treatment teams, fostering growth and utilizing incentives.
F) Coordinate services for contacts monthly, emphasizing person-centered approaches and promoting support network development.
G) Enroll eligible individuals into IOS programs, maintain communication, and coordinate appointments.
H) Refer contacts to appropriate community providers, facilitating connections.
I) Provide transportation adhering to traffic laws and utilize SAMA methods during crisis situations.
J) Complete progress notes for each contact and file them in their chart. Track services and referrals on spreadsheets, submitting weekly updates to the supervisor.
K) Enter data into HMIS / ETO, CMBHS, and Netsmart NX : Input all contact services into Netsmart NX and HMIS / ETO for those experiencing homelessness. Document actions moving contacts through change stages, discussing achievements regularly.
L) Interact positively with contacts and organizations, using proper communication channels.
M) Maintain positive interactions to prevent negative outcomes for contacts and organizations.

O) Performs other job duties or responsibilities as requested or assigned.
III) Knowledge of Laws, Regulations, Policies / Procedures, Skills, and Abilities
A) Demonstrate training ability for HIV / STI, HCV, mental health, substance use, and homelessness.
B) Work independently, complete training, and exercise sound judgment.
C) Interact courteously and represent MHMR of Tarrant County professionally.
D) Perform job duties within schedule, fostering relationships with clients.
E) Possess knowledge of client needs, assess, and create treatment plans.
F) Navigate community resources, make productive referrals, and resolve conflicts.
G) Approach clients non-judgmentally, maintain contact with service providers.
H) Communicate clearly in verbal and written forms.
I) Understand human development, psychology, and substance abuse.
J) Contribute effectively to treatment teams, accept professional supervision.
K) Understand medication classes, side effects, and practice effective skills.
L) Familiarity with Medicaid and community standards, and payer requirements.
M) Knowledge of relevant policies, procedures, and ethical guidelines.
N) Complete required organizational and grant-related training.
IV) Internal & External Customer Service
A) This position requires extensive internal and external contacts. The employee will accomplish this with advanced written and verbal skills.
B) Must demonstrate compassion, trauma-informed care, and safety practices including suicide safe care.
V) Travel
A) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.
B) This position involves occasional overnight statewide travel, as well as potential County-wide travel up to 100% of the time, with the possibility of using a personal vehicle for travel between service locations.
VI) Equipment Used
A) Utilized as required for position.

A) Minimum Education : High School Diploma / GED
B) Defined Education : None
C) Preferences : None
D) Substitutions : Associate Degree in health and human services field with no prior experience
E) Years' Experience : Two (2) years
F) Defined Experience : Community-based experience on projects working with serious mental illness, substance use disorders, HIV / STI prevention and testing, persons experiencing homelessness, HCV prevention and testing, and community advocacy, work / life experience.
G) License / Certifications : Valid Texas Driver's License, Certified Community Health Worker (within 6 months of hire date), Recovery Coach Certification (withing 60 days of hire date)
H) Special Courses : None
I) Supervisory Experience : None
VIII) Agency Requirements
A) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key / essential staff level function during critical events or for the purpose of sustaining business continuity.
B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and / or the Division / Director.
C) Assigned work hours may change as the needs of the agency change.
D) The Functional Title of this position may change as the needs of the agency change.
E) All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.
F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.
G) Employment is at will, as well as agency needs may change.
H) Agency dress code is to be followed at all times.
I) Physical on-site presence, including regular attendance and punctuality, is an essential function of this position. Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
