Employment Type: Full time Shift: Day Shift Description: *$10k sign on bonus- Above and Beyond Health at Home Care- Monticello is seeking a compassionate and dedicated Home Care Registered Nurse (RN) to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring they receive the support they need to maintain their health and independence. Key Responsibilities: Conduct comprehensive assessments of patients' health status and needs. Develop and implement individualized care plans in collaboration with interdisciplinary teams. Administer medications and treatments as prescribed and monitor patient responses. Educate patients and families on disease management, medication usage, and health maintenance. Monitor vital signs and manage chronic conditions, ensuring timely interventions. Document patient care activities accurately and in a timely manner. Coordinate care with physicians, therapists, and other healthcare professionals. Provide emotional support and counseling to patients and families. Ensure compliance with all healthcare regulations and standards. Participate in quality improvement initiatives and ongoing education. Qualifications: Current Registered Nurse (RN) licensure in the state of Iowa. Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) acceptable. Previous home health or community nursing experience preferred. Strong clinical assessment and critical thinking skills. Ability to work independently and manage time effectively.
