As part of Trinity Health at Home, you will have the opportunity to make a positive impact by providing care to patients in the comfort of their homes. You will deliver high-quality therapy, design individualized treatment plans, and support patients in regaining independence and mobility. Responsibilities: Assess, plan, and administer physical therapy treatments for patients in their homes. Develop personalized treatment goals based on patients' individual needs. Educate patients and caregivers on therapeutic exercises and safe mobility practices. Coordinate with other healthcare team members to ensure comprehensive patient care. Document treatments and progress accurately in accordance with regulatory and company standards. Requirements: Current State of Michigan licensure as a Physical Therapist. Minimum of one year of experience in physical therapy, preferably in a home health or related setting. Strong communication and organizational skills. Ability to work independently and manage a caseload in a home setting.
