Minimum Qualifications Education & Licensure: Bachelor’s Degree in Nursing (BSN) or equivalent experience as a Registered Nurse (RN). Current nursing license in the state of employment. Certified Registered Nurse Infusion (CRNI) credential preferred or willingness to obtain. Experience: At least two (2) years of acute care nursing experience. Sales or marketing experience is a plus. Skills & Requirements: Excellent customer service and communication skills (oral and written). Valid driver’s license and reliable transportation. Job Description: Home Infusion Coordinator Essential Functions: Regulatory and Ethical Compliance: Ensure adherence to Federal, State, local laws, and agency policies, maintaining integrity and professionalism. Coordination of Care: Collaborate with case managers and discharge planners to facilitate timely and efficient home infusion services. Patient Assessment and Planning: Assess client needs, develop service plans, and coordinate with physicians and healthcare teams. Education and Support: Provide patients and families with comprehensive education on home infusion services. Interdisciplinary Collaboration: Work with other healthcare providers to ensure effective care and make appropriate referrals. Documentation and Communication: Accurately document assessments, interventions, and client interactions; maintain ongoing communication with healthcare teams. Quality Improvement: Actively participate in quality initiatives and assist in developing new programs to meet patient needs. Sales and Relationship Building: Achieve sales targets, foster positive relationships with healthcare professionals, and promote agency services. Professional Development: Maintain CRNI credentialing, mentor colleagues, and stay current with best practices. Mental and Physical Requirements: Frequent standing, walking, and lifting (35%). Occasional bending, stooping, twisting, and positioning patients (10%). Adaptability to reprioritized tasks and frequent interruptions (90%). Travel to various locations as needed.
