Job Title : Activities Coordinator
Job Summary
Under the supervision of Pace Site Administrator, has full responsibility for all daycare and activities throughout the adult day center; The Activity & Day Center Coordinator manages the daily planning and coordination of center-based activities to include conflict resolution, participant / caregiver care issues, certified nursing assistance and therapeutic program.
Essential Job Functions
- Conducts initial assessment and scheduled reassessment of participants
- Develops specific therapeutic programs to include recreational and social activities
- Records, signs, and dates progress reports in participant records
- Maintains the daily operation of the day center
- Plans and provides both group and individual programming
- Participates in interdisciplinary meetings and developing individualized and measurable care plans
- Supervises with participant care needs such as toileting and feeding
- Supervises day care certified nursing assistants, volunteers and additional daycare staff
- Responsible for scheduling staff, toileting schedule and participant tracking record
- Keeps inventory and order sheets for recreation and day center supplies
- Plans and administers participant council
- Completes other assignments as needed
