We are now looking for skilled and compassionate LPNs to join our team in providing exceptional care for individuals receiving mental health and substance abuse treatment in our residential crisis stabilization community.

Must be available for weekend shifts: Saturdays AND Sundays 9A-3P &/or 3P-11P

What You’ll Do:

As an LPN on our team, you will work closely with other healthcare professionals to assist patients in achieving stabilization and reintegration. Your primary responsibilities will include:

Patient Assessment: Regularly assess and document residents' health status, medication compliance, and response to treatment. Monitor any changes or concerns.

Medication Management: Administer prescribed medications, monitor side effects, and educate residents about their medications and health management strategies.

Collaboration with Healthcare Team: Work with physicians, nurses, counselors, and other professionals to coordinate individualized care plans and participate in treatment team meetings.

Health Education: Provide education on wellness, nutrition, and the effects of substance abuse. Lead individual and group sessions to promote healthy lifestyles.

Emergency Response: Respond to medical emergencies and coordinate care with external medical services when necessary.

Documentation: Maintain accurate and up-to-date medical records in accordance with regulatory requirements.

Care Coordination: Connect patients with clinical and social resources, supporting discharge planning and follow-up care.

Compliance: Ensure adherence to regulatory requirements and participate in quality assurance activities to improve care.

Safety: Provide a safe and secure environment for crisis intervention.

Qualifications:

• Successful completion of an accredited LPN program
• Active, unencumbered LPN license in Virginia
• Current CPR/First Aid Certification
• Experience in behavioral health/psychiatric/addiction recovery preferred
• Strong interpersonal and communication skills
• Ability to work collaboratively in a fast-paced environment
• Commitment to trauma-informed, patient-centered care

Working Conditions:

• Shift work: Weekend Day &/or Evening Shifts
• Saturdays AND Sundays 9A-3P &/or 3P-11P
• Adherence to infection control protocols and safety measures is essential
