What are the responsibilities and job description for the Nurse Care Coordinator position at Micah Ecumenical Ministries?
The Nurse Care Coordinator is responsible to the Health Innovation Coordinator for providing health-focused case management to Micah’s currently and formerly-unhoused neighbors. This position works alongside housing, shelter and street case managers, with a specialized knowledge for assessing, arranging, coordinating, and monitoring the delivery of individualized health services for the purpose of achieving and/or maintaining housing stability. The end goal is increasing access to healthcare services and supports that address neighbors’ physical and mental well-being, and thus increase stability in housing.
Responsibilities:
Care Coordination
- Assist neighbors and Micah staff supporting them (from various Micah ministries) in navigating health resources, including referrals to HealthBridge.
- Provide support for success planning as it relates to individual neighbors throughout the organization with specific focus on their health needs as related to housing stability.
- Assist neighbors and staff supporting them in managing the healthcare needs of chronic or serious conditions such as diabetes, heart disease, dementia, and cancer.
- Advocate for personalized care options that address a neighbor’s unique needs.
- Assist the Health Innovation Coordinator to schedule the neighbors HealthBridge appointments and follow up with their Micah Care Manager to ensure attendance.
- Communicate and educate neighbors and staff supporting them regarding their health condition(s)
- Offer education and guidance for navigating complex medical decisions.
- Serve as a liaison between neighbors and their healthcare providers to promote quality, cost-effective care with the best patient outcomes.
- Facilitate credentialed documentation of disabilities with healthcare partners, as appropriate.
Community Engagement
- Work with community-based health partners, including but not limited to primary care providers, mental health counseling, psychiatry, skill builders, nursing homes and in-home care providers, to support partners that increase access to services and resources that address the physical and mental well-being of neighbors.
- Participate in health education of Micah staff and neighbors, with priority for learning preventative care and avoidance of the emergency room for basic medical/mental health needs.
- Work with Micah volunteers in supporting the health needs of Micah’s neighbors.
Administration
- Participate in Health Ministry data collection and reporting related to health services.
- Participate in documentation of care and summary of appointments with HealthBridge healthcare providers in HMIS, Monday.com and other appropriate platforms for effective communication of neighbor’s healthcare status.
Team Participation
- Participate in staff meetings and other group activities essential for operations
- Participate in team discussions related to health issues and help to develop possible solutions to ensure best support for the client’s success.
- Promote good community relations and utilizes community services and resources.
- Attend scheduled training programs for professional development that includes, at a minimum, trainings required by Micah and by regulatory and accrediting bodies.

Knowledge and Skills:

Qualifications and Skills
Minimum Registered Nurse with Associate of Science in Nursing or Bachelor of Science in Nursing with at least two years working with a special needs population (i.e. homeless, mentally ill, substance use, etc.) or commensurate experience or training. Demonstrated knowledge of skills in behavioral health, treatment planning, community rehabilitation and case management. Must have an understanding of relationship-based service models and be comfortable working alongside volunteers. Must be a self-initiator and be able to problem solve various life issues. CPR certification required at hiring.
Supervision:
The Nurse Care Coordinator will be supervised by the Health Innovation Coordinator. Performance evaluations will be completed on an annual basis.
Work Schedule:
This position is classified as a part time for 15-20 hrs/week; flexibility in hours/days to accommodate other partners is a necessity.
