Responsibilities*
As a member of a multidisciplinary team, the RN Clinical Care Coordinator will provide quality care to patients, caregivers and donors who are followed by the Immunohematology group. This population includes patients across the lifespan receiving comprehensive care to treat immune deficiencies, bone marrow failure syndromes, lymphoproliferative diseases, and neutrophil disorders.
The Clinical Care Coordinator, in collaboration with members of the multidisciplinary team, will practice across settings and the continuum of care to ensure continuity of care for patients and families. Job responsibilities include supporting the clinical management of patient referrals, seeing patients, and conducting physical assessments, maintaining communication about patients being seen in our program. They will assist in creation and implementation program documentation for patient care and conduct educational sessions with patients & caregivers for the many phases of care encompassed in this care process.
The Clinical Care Coordinator will be required to learn and maintain compliance with current therapies to meet all standards, audits, and regulatory requirements. The coordinator will need to maintain REMS certification. They will enroll patient in mandatory FDA tracking system if required, schedule diagnostic evaluations, and maintain communication with pharmaceutical personnel regarding status of commercial product.
The Clinical Care Coordinator will assist the program faculty, administrator, and the regulatory/quality manager in the development of best practice guidelines for clinical care in preparation for program accreditation. They will attend the weekly clinical operations meeting where patients are discussed with multi-disciplinary team. They will assist in the maintaining a list of new and current patients.
The Clinical Care Coordinator will work closely with research coordinators to develop necessary workflows to provide the best experience for patients who enroll in clinical trials. Participate in workshops and CME activities pertinent to the Immunoheme population.
Attendance at programmatic meetings at Michigan Medicine, and occasionally attend regional or national meetings.

Bachelors degree in Nursing - OR - Associates degree or diploma in Nursing and a Masters degree in Nursing
Current license to practice nursing in the State of Michigan.

- Deliver expert, accurate disease and therapy related information to both the patient and family to reinforce and expand knowledge gained from other clinical providers to empower patients and caregivers with the ability to confidently assume responsibility for their care.
- Serve as an active liaison for the patient/family between multiple sub-specialty services and assist with coordinating an integrated treatment approach across the healthcare continuum to minimize redundancy and inefficiencies in care. The care coordinator will work closely with Genetics, Rheumatology, and Hematology services.
- Assist with optimizing the scheduling of follow up visits and studies for a more ideal patient/family experience by recognizing both routine disease-specific surveillance needs and ad hoc urgent needs across service lines to facilitate bundled care.
- Provide trusted emotional support screening for patients/families with immune and blood related disorders and serve as a bridge to other members of the health care team, e.g. social work, psychology, and/or nutrition, as well as disease-specific patient advocacy groups to provide multi-faceted support for the patient and family.
- Function as a patient advocate to identify patients eligible for additional services, e.g. genetic testing/counseling, fertility preservation, financial support programs and clinical trials as appropriate.
