Occasional in-person at Lansing, MI office required
Purpose : This position engages with the Michigan Department of Health and Human Services (MDHHS) public health navigators and Michigan residents toprovide resources to reduce potential lead exposure for community members and their families. The operators will understand existing MDHHS program resources and complete referrals or schedule MDHHS program services for community members. This position will be housed in the MDHHS Community Services (CS) Unit, within the Division of Toxicology and Response under the Environmental Health Bureau in Lansing.
Duties and Responsibilities :
- Communicate with MDHHS public health navigators and community members, providing MDHHS program resources, scheduling appointments, and performing phone outreach to share program qualifications and referrals.
- Participate in scheduled meetings (remotely or in-person) with MDHHS and partner agencies.
- Accurately complete and submit documentation / forms on referral and scheduling activities and outcomes.
- Assist with program-related projects and activities as needed.
- Conveys and follows up on assignments to navigator staff.
- Answers scheduling and results request calls and emails and directs inquiries to appropriate staff.
- Maintain a positive, strong, credible, professional, and interpersonal relationship with all parties relevant to MDHHS and Michigan Public Health Institute (MPHI) projects and always represent the best interests of MDHHS and MPHI.
- General administrative duties as needed.
Qualifications / Requirements :
Education : Possession of a high school diploma or equivalent required. Preferred : Associate's degree in communications, public health, community health services, health promotion or community outreach / engagement.
Experience : 1 or more years of experience conducting referrals and care coordination including case management and / or working with residents to provide MDHHS program resources, identify support needs, and problem solving is preferred. 1 or more years working with multiple software platforms, scheduling, spreadsheet development, and data processing is preferred.
Important Skills and Characteristics :
Work Environment and Physical Requirements : Physical requirements consistent with a standard office environment. Items and supplies can be bulky and weigh up to 40lbs.
Occasional attendance at meetings and events outside of the assigned workspace will be required. Some activities or events may be held outside of typical business hours or on weekends.
Travel : This position may require occasional travel to Lansing, MI to meet with team.
Responsibility for the Work of Others : No assigned responsibility.
