IOP Clinician- PT

Clinician will provide intensive, multi-modal treatment (e.g. individual, family, group, and multifamily therapy group) and stabilization for children/adolescents presenting with complex behavioral health needs due to psychological reactions to trauma, anxiety, or depression in different environments.

DUTIES AND RESPONSIBILITIES

1. Conduct comprehensive evaluations and assessments to generate clinical formulations that inform individualized treatment plans for clients on your clinical caseload.

2. Carry clinical caseload with clients from the Prospects Extended Day Treatment Program and the Intensive Outpatient Program; the number of cases will be determined by your supervisor.

3. Provide individual, family, group, and multi-family therapy group to clients on your clinical caseload.

4. Coordinate intake process and discharge plans for clients on your clinical caseload.

5. Provide risk assessment and crisis interventions, as needed.

6. Maintain all necessary electronic health records and scheduling requirements per compliance regulations.

7. Obtain and maintain insurance authorizations.

8. Assist in making transportation arrangements, as needed

9. As available, attend psychiatric evaluations and medication management appointments with psychiatry team for clients on your clinical caseload.

10. Attend and participate in all necessary meetings including but not limited to interdisciplinary treatment plan review meetings, clinical rounds, community activities/events, staff meetings, conferences, in-service training and workshops identified by Mid Fairfield Community Care Center as conducive to continued growth and skill development.

11. Collaborate with outside community and or school-based providers to ensure continuity of care for clients on your clinical caseload.

12. Attend Planning and Placement Team Meetings, 504 Team Meetings, and/or any other school meetings as a representative for clients and the EDT program, as warranted

13. Participate in after-hour, on-call (telephone consult only) rotation at a rate of one time, every 7-10 weeks.

REPORTING RELATIONSHIP

This position reports to the supervisor named in the letter of appointment, and is accountable to this supervisor for the performance of all professional activities at Mid Fairfield Community Care Center. The clinician is a member of the interdisciplinary team.

Qualifications


QUALIFICATIONS

1. Master’s Degree or higher from an accredited school of Social Work, Marriage and Family Therapy, Counseling or Psychology Program

2. Must have Connecticut State License or Associate License in Social Work, Marriage and Family Therapy, Counseling, or Psychology

3. Minimum of two-years’ experience providing individual, family, and group therapy to children with complex behavioral health needs (and their families)

4. Experience with risk assessment and crisis management skills

5. Computer skills, including Google Workspace and Microsoft Office; familiarity with Electronic Health Records Systems a plus

- Proven ability to follow program and agency ,protocols as well as making independent decisions that are in the best interest of the families while working both in the office and in the field

- Proven ability to communicate with families as well as with other providers such as school social workers, teachers and other community partners among others

- Demonstrated ability to maintain healthy boundaries between personal and professional responsibilities to be able to function in a high stress job

- Proven ability to manage conflict appropriately and advocate for victims of violence

- Family Engagement:

- Knowledge of person centered care

- Appropriate guidance and facilitation skills for families in the context of their culture, health status, and health needs

- Technical:

- Knowledge of basic group skills o Basic negotiation skills

- Ability to delegate tasks, responsibilities, etc.

- Excellent time management skills
